One-time $99 enrollment fee covers your intake, history review, goal setting, and baseline lab review. No annual contract.

Included

  • Provider access through scheduled telehealth visits and secure non-urgent messaging, within your tier.
  • Routine primary-care support appropriate for telehealth, prescription management within scope, and care and referral coordination.
  • A continuing provider relationship: your goals, medication history, and progress are followed over time.
  • Preferred pricing on cash-pay labs, wellness services, and separately billed peptide, GLP-1, hormone, and supplement programs.

Not included

  • Hospital, emergency department, ambulance, specialist, imaging, laboratory, medication, compounded medication, peptide, GLP-1, or supplement costs, unless stated in writing.
  • Emergency or continuously monitored after-hours care. Call 911 for urgent or life-threatening symptoms.
  • Services that need an in-person exam, testing, or a higher level of care than telehealth can safely provide.
  • Unlimited extended functional-medicine visits or medication-specific protocol monitoring beyond your plan's stated benefits.

Membership terms

  • Apex Direct is a cash-pay practice. We do not bill insurance or submit claims on your behalf.
  • Membership is not health insurance and does not replace comprehensive major medical or catastrophic coverage. Keeping that coverage is strongly recommended.
  • Each enrolled adult and child has a separate enrollment, consent, health history, and medical record. A parent or legal guardian must enroll and take part in care for a minor.
  • Memberships bill monthly. No annual contract; cancel with 30 days' written notice.
  • A one-time $99 enrollment fee applies to new memberships and covers the initial health intake, history review, goal setting, and baseline lab review when available.
  • Labs, medications, compounded prescriptions, peptides, GLP-1 products, and outside services are billed separately unless a written program states otherwise.
  • Telehealth is used only when your provider determines it is clinically safe. Some concerns need in-person or emergency evaluation.
